The Continuum Model 

The Collaborative follows our continuum model which includes innovative features to help kids if they're at-home or living with us and getting ready to go home.

The model which includes the use of an assigned ‘core team’ allows clinical staff to provide true continuity of care by working with the youth and family regardless of the child’s living situation. This means avoiding the dreaded “start over” with a new service provider if a child is transitioning between home and out-of home treatment. Regardless of the child’s level of care the unified treatment plan and ‘core team’ remains the same.

The Continuum Model also includes the use of occupational therapy and a peer mentor to support youth and their families. Clinical approaches and the wide variety of services offered prepare youth for a smooth transition home and give them the tools they need to be safe.
